On-line Pre-Arrangement Form
Each death in New Zealand must be registered with the Central Registry Office within three working days of the burial or cremation. The first part of this form lists the information we require to be able to register that death in New Zealand, it is also the information that will appear on the Death Certificate. The second part of this form lists individual wishes for the funeral service.
